Function Chiropractic follows all federal HIPAA regulations to protect your personal health information (PHI). Your health information will not be shared, sold, or disclosed without your written authorization, except when permitted or required by law. You have the right to access your health records, request corrections, and receive an accounting of disclosures. Full HIPAA policies are available upon request at our office.

Under the federal No Surprises Act, you are entitled to receive a Good Faith Estimate for the total expected cost of any non-emergency services. This includes all fees related to chiropractic treatment. If you are uninsured or choose not to use insurance, you have the right to request a Good Faith Estimate prior to scheduling. Estimates are based on typical care plans and may vary depending on your individual response to treatment.
Due to federal regulations, Medicare, Medicaid, TriCare, and all other government-funded insurance programs are excluded from any promotional offers, discounts, packages, or reduced-rate services, including but not limited to new patient specials or bundled pricing. Patients with these insurances will be billed according to federal rules and coverage limitations. Certain services, including exams, therapies, and maintenance care, may not be covered by Medicare or government plans.
Promotional offers, discounts, new patient specials, and cash-based packages cannot be used for Workers’ Compensation, Personal Injury, auto accident, liability, or third-party insurance cases. These cases follow separate legal and billing guidelines, and services must be processed according to carrier requirements.
We strive to provide clear pricing and coverage expectations before treatment begins. Patients using BCBS PPO insurance will receive a breakdown of deductible, copay, and coinsurance information after benefits are verified. Cash prices are available for patients who do not wish to use insurance. Payment is due at the time of service unless otherwise required by insurance rules. A card on file is required for all patients for scheduling and cancellation compliance.
By providing your contact information, you consent to receive appointment reminders, practice updates, and essential communication regarding your care. Patients will be asked for explicit, written consent before any photos or videos are used for marketing, testimonials, or educational purposes. Recording or media capture will never take place without prior notification.
